About the position

The Community Support Specialist - Case Manager for Children/Adults at Swope Health Services plays a crucial role in providing direct services to individuals with severe emotional disturbances or serious mental illnesses. This position focuses on supporting individuals to maintain and improve their community living situations through effective case management, resource connection, and personal development. The role requires strong communication and interpersonal skills, as well as a commitment to serving the community and enhancing the well-being of clients.

Responsibilities

  • Provide case management services in the community, connecting individuals to resources and arranging access to rehabilitation groups, medical care, education, and psychiatric care.
  • Complete documentation of services provided following guidelines from Missouri Medicaid/Department of Mental Health and enter into the electronic health record in the timeframe allowed.
  • Work with individuals to set goals and develop a treatment plan to measure their progress toward goals.
  • Participate in annual assessments for continuation of services.
  • Assist with housing needs and coach individuals on life skills to improve activities of daily living.
  • Participate in discharge planning for individuals that have been hospitalized.
  • Provide crisis intervention during working hours and referral for crisis after hours.
  • Assist with application and renewal of benefits like Medicaid, SNAP, SSI/SSDI, and Ticket to Work.
  • Participate in psychiatric and medical appointments to assist with achieving overall health and wellness goals.
  • Provide transportation as medically necessary for individuals to access resources or attend community appointments.
  • Follow agency policies and procedures and perform other duties as assigned.

Requirements

  • High School diploma or GED required with appropriate years of experience working with individuals with psychiatric, substance use, or developmental disabilities.
  • Associate's or Bachelor's degree in a Human Services related field preferred.
  • Valid Driver's License and Insured Automobile.
  • Flexibility and adaptability in work.
  • Knowledge of community resources and how to access them.
  • Ability to build relationships and rapport with individuals.
  • Desire to serve others and carry out the mission of Swope Health.
  • Ability to write and communicate effectively.
  • Demonstrate cultural competency with a diverse patient population.
