About the position

The Community Support Specialist (Case Manager) at Heritage Behavioral Health Center plays a crucial role in providing case management, care coordination, and linkage services for individuals with mental health, substance use, and co-occurring disorders. This position is dedicated to supporting individuals in maintaining recovery within the community, working collaboratively with treatment teams to deliver services in various settings, including individual homes and community agencies.

Responsibilities

  • Provide case management and community support services while managing individual caseloads.
  • Engage clients through telephone and face-to-face interactions, primarily in community settings.
  • Complete needs assessments to identify additional needs related to mental health, substance use, health, and other support systems.
  • Assist clients in developing coping skills to manage their symptoms effectively.
  • Support clients in learning independent living skills, including meal preparation and budgeting.
  • Provide support services to help individuals obtain and maintain stable housing.
  • Monitor medication and provide training on self-administration and symptom identification.
  • Coordinate referrals and linkages to healthcare services, including primary and specialty care.
  • Assist individuals in applying for and maintaining resources like medical coverage and SNAP benefits.
  • Advocate for clients to access housing, benefits, and treatment, seeking reasonable accommodations when necessary.
  • Coach clients on life skills and assist in developing positive social activities that support recovery.
  • Engage in a team-based approach to treatment, collaborating with various healthcare providers and partners.
  • Attend clinical staffing to coordinate treatment with other professionals.
  • Complete documentation and tracking of services provided.

Requirements

  • Five years of experience in a behavioral health setting, OR Certified Recovery Support Specialist (CRSS), OR a Bachelor's Degree in psychology, social work, human services or related field, OR a Bachelor's Degree in any field with two years of experience in a human service-related setting.
  • Ability to work with individuals with mental health and/or substance use diagnoses.
  • Knowledge of community resources and case management techniques.
  • Ability to build rapport quickly and work as a team player.
  • Ability to navigate electronic health records and respond to supervisors and clinical staff.
