About the position

ALM Hopewell Center is dedicated to providing comprehensive mental health services to children and adults in the northern and central parts of the City of St. Louis and North St. Louis City. The organization serves a growing caseload of 2,800 patients, with over ninety-five percent of these individuals being chronically mentally ill and suffering from major psychiatric disorders. The center operates both a Psychosocial Rehabilitation Center and a Comprehensive Psychiatric Rehabilitation Center for adults and youth, with many services being provided at schools. Emergency services, including a crisis bed and mobile outreach, are available twenty-four hours a day. A multi-disciplinary team consisting of psychiatrists, psychologists, nurses, counselors, social workers, educators, and support staff work diligently to fulfill the mission of providing comprehensive care to patients and their families. Currently, Hopewell Center has openings for Community Support Specialists (CSS) in the Children Services Department, which caters to individuals aged 6-18 years. The CSS is responsible for coordinating community resources and facilitating interactions that support individuals at clear risk of hospitalization, alongside providing school-based and family support services. This role involves transporting clients to appointments and making school and home visits. Additionally, the CSS works with individuals who have been discharged from inpatient treatment to assist them in adjusting to community living. The CSS is tasked with evaluating clients' statuses, identifying personal strengths, areas of concern, and needs. They will participate in formal assessments and the development of person-centered plans, monitor clients' progress towards goal attainment, and provide assistance as a 24-hour resource when necessary. Training clients in living skills, social skills, and other pro-social activities is also a key responsibility, enabling clients to actively participate in their recovery process. The CSS is an integral member of the Clinical Treatment Team and is responsible for maintaining client records while ensuring that standards and guidelines are upheld.

Responsibilities

Requirements

  • Bachelor's degree in Social Work, Psychology, or other behavioral health sciences related field.
  • Associates Degree with 2 years of experience in Community Mental Health, Case Management, Intensive In-Home services, or Social Work.
  • 4 years of experience in Community Mental Health, Case Management, Intensive In-Home services, or Social Work.
  • Valid Class E driver's license, dependable transportation, and personal vehicle insurance.
