About the position

The Community Support Specialist Crisis Integration plays a crucial role in providing outreach and coordinating care for clients utilizing the Crisis Access Points within the Compass Health Network. This position is essential for ensuring that clients receive the necessary support and resources during crises, particularly in the Western region of the network. The specialist will work closely with various programs and clients to facilitate access to care and assist with their basic needs.

Responsibilities

  • Coordinates with CPRC, outpatient and crisis services for all active CPRC or YCPRC clients referred to ERE/YERE or seen at Crisis Access Points.
  • Meets with CPRC clients at the Crisis Access Points to offer additional support for those needing a higher level of care.
  • Assists clients in crisis by coordinating with all CHN programs, helping with basic needs, attending appointments, and obtaining resources.
  • Performs outreach and follow-up with clients as required in person.
  • Transports CPRC clients to the CAP as needed.
  • Ensures all ERE/CAP CPRC monthly chart reviews are completed thoroughly and in a timely manner.
  • Attends ERE/CAP/integrated meetings.
  • Works with clients to identify barriers to participating in their current programs.
  • Completes hospital follow-up documentation with CPRC/YCPRC clients as needed.
  • Reconnects and re-engages CPRC clients with their providers.
  • Reviews regional CAP and ERE spreadsheets and referrals daily for CPRC referrals.
  • Completes CAP and ERE data for all CPRC clients referred.
  • Documents all coordination of care/services within 24 hours.
  • Driving is an essential function of this position.

Requirements

  • High School/GED required.
  • Associate's/Bachelor's degree in a human service-related field preferred.
  • At least four (4) years of experience OR a combination of higher education and experience required if a High School/GED is the highest education obtained.
  • At least two (2) years of experience OR a combination of higher education and experience required if an Associate's degree is the highest education obtained.
  • At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities.
  • A valid driver's license and agency established minimum automobile coverage required.

Nice-to-haves
