About the position

Pine Belt Mental Healthcare Resources is seeking a dedicated and energetic Crisis Community Support Specialist to provide intensive case management services to adults with a serious mental illness diagnosis as part of a team in McComb, MS. This position is crucial in helping individuals maintain emotional and mental stability while developing a more satisfying and productive life. The Crisis Community Support Specialist will engage with each individual as frequently as necessary to ensure their stability and progress. This role involves assisting individuals in setting goals and working towards achieving specific objectives, which is essential for their recovery and integration into the community. The Crisis Community Support Specialist will be responsible for medication monitoring and system navigation, ensuring that clients receive the necessary support and resources. They will also assist in outreach efforts for the program and case management, demonstrating a thorough understanding of various diagnoses, signs and symptoms of relapse, and medication information. Knowledge of community resources is vital to explore all alternatives to hospitalization, thereby providing a holistic approach to care. In addition to direct client services, the specialist will facilitate Person Centered Planning meetings, provide advocacy, consultation, and educational services, and link clients to outpatient social services. Emergency intervention and crisis intervention services will also be part of the responsibilities, ensuring that clients receive immediate support when needed. The position requires home or community visits to provide services, emphasizing the importance of accessibility and personalized care in the community.

Responsibilities

  • Assists with medication monitoring and system navigation.
  • Assists with outreach of program and case management.
  • Demonstrates knowledge of diagnosis, signs and symptoms of relapse, medication information, and knowledge of community resources to explore all alternatives to hospitalization.
  • Facilitates Person Centered Planning meetings.
  • Provides advocacy, consultation, and educational services.
  • Provides outpatient social services linkage, emergency intervention, and crisis intervention services.
  • Provides services to clients through home or community visits.

Requirements

  • Bachelor's degree in behavioral healthcare required
  • Must have reliable transportation to provide community services
  • Must have a valid driver's license with a satisfactory driving record
  • Must be able to obtain DMH case management certification within 60 days of hire
  • Must have auto insurance liability limits of 100,000/300,000/50,000 within 1 month of hire
  • Behavioral healthcare experience preferred
  • Excellent communications skills required
