About the position

The Community Support Specialist - Disease Management is responsible for conducting outreach and coordinating care for active disease management clients. This role involves engaging with clients through in-person and phone outreach, ensuring they receive the necessary support and services to manage their health effectively. The specialist will work closely with various teams to facilitate care transitions and document all interactions to maintain accurate records.

Responsibilities

  • Conducts in-person and phone outreach to all active disease management clients monthly.
  • Sends outreach letters to all active disease management clients.
  • Coordinates with the Behavioral Health team on all disease management clients who are incarcerated.
  • Conducts outreach to disease management clients who are currently hospitalized and attempts to coordinate care with the hospital.
  • Confirms client Medicaid status, recent hospital stays, ER visits, medications, contact information, and pharmacies.
  • Coordinates with internal therapy, psychiatric, and health resource specialists to transition clients to community support services.
  • Works with clients to identify barriers to enrollment in services.
  • Attends integrated meetings in offices where Community Access Points (CAPs) are located to coordinate care for active or inactive disease management clients.
  • Documents all coordination of care and client interactions within 24 hours.
  • Maintains a tracking system for all clients needing contact each month.
  • Coordinates with Health Access Liaisons (HALs) to obtain virtual assessment appointments for disease management clients as requested.
  • Attends all ERE and HAL meetings and huddles for full integration into each team.
  • Collaborates monthly with the disease management coordinator on cyber/active client lists to ensure they are up to date.
  • Inputs disease management data into Care Manager as needed.
  • Completes monthly chart reviews on cyber monitoring clients.

Requirements

  • High School diploma or GED required.
  • Associate's or Bachelor's degree in a human service-related field preferred.
  • At least four (4) years of experience or a combination of higher education and experience required if a High School/GED is the highest education obtained.
  • At least two (2) years of experience or a combination of higher education and experience required if an Associate's degree is the highest education obtained.
  • At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities.
  • A valid driver's license and agency established minimum automobile coverage required.

Nice-to-haves
