The role requires daily interaction with clients, including home visits, and emphasizes the importance of building relationships to promote recovery and resilience.

Responsibilities

• Provides community support services to complex clients, utilizing treatment plan interventions for positive outcomes.
• Completes timely documentation of services, detailing the need for service, interventions provided, and client responses.
• Contacts individuals or referral sources after missed appointments to re-engage clients and support recovery efforts.
• Mentors and provides job shadowing to community support colleagues.

Requirements

• Bachelor's Degree in Human Services or a related field.
• Valid Class D (IL) or Class E (MO) driver's license.
