About the position

The Community Support Specialist for the Emergency Room Enhancement program at Compass Health Network plays a crucial role in engaging consumers into services and assisting them in accessing the necessary resources to achieve and maintain their health and recovery. This position is vital due to the rapid expansion of services across all clinics, and it aims to ensure that clients receive the support they need in a timely manner. The Community Support Specialist will work collaboratively within a team environment, contributing to a culture that values inclusivity and support for both clients and staff. The role involves providing individual assistance to consumers in navigating mental health and medical services, as well as public services such as financial and housing assistance. This includes emergency support to meet immediate needs for food, shelter, and clothing. In addition to direct assistance, the Community Support Specialist will help consumers access and utilize various community agencies to acquire ongoing social, educational, vocational, and health resources. Training and coaching in daily living skills are also key components of this role, which includes helping individuals with housekeeping, cooking, personal grooming, budgeting, and maintaining an independent residence. Follow-up with consumers regarding appointments and necessary documentation is essential to ensure continuity of care and support. The position may also involve other duties as assigned, contributing to the overall mission of Compass Health Network to inspire hope and promote wellness in the communities served.

Responsibilities

  • Provide individual assistance to consumers in accessing needed mental health and medical services
  • Assist consumers in accessing a variety of public services including financial and housing assistance
  • Help meet emergency needs for food, shelter, and clothing
  • Assist consumers in utilizing community agencies for ongoing social, educational, vocational, and health resources
  • Train, coach, and support consumers in daily living skills such as housekeeping, cooking, and personal grooming
  • Follow up with consumers regarding appointments and completion of necessary forms
  • Perform other duties as assigned

Requirements

  • High School/GED required
  • Associate/Bachelor's/Master's degree in a human service-related field preferred
  • At least four (4) years of experience or a combination of higher education and experience required if a High School/GED is the highest education obtained
  • At least two (2) years of experience or a combination of higher education and experience required if an Associate's degree is the highest education obtained
  • At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained
  • All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities
  • A valid driver's license and agency established minimum automobile coverage required

Nice-to-haves

  • Experience in a nonprofit health care organization
