About the position

The Community Support Specialist for the Emergency Room Enhancement program at Compass Health Network is responsible for engaging consumers in services and assisting them in accessing necessary resources to achieve and maintain health and recovery. This role emphasizes collaboration within a team environment to provide quality care and support to individuals in need.

Responsibilities

• Provide individual assistance to consumers in accessing needed mental health and medical services
• Assist consumers in accessing a variety of public services including financial and housing support
• Help consumers meet emergency needs for food, shelter, and clothing
• Assist consumers in utilizing community agencies for ongoing social, educational, vocational, and health resources
• Train, coach, and support consumers in daily living skills such as housekeeping, cooking, and budgeting
• Follow up with consumers regarding appointments and completion of necessary forms
• Perform other duties as assigned

Requirements

• High School/GED required
• Associate/Bachelor's/Master's degree in a human service-related field preferred
• At least four (4) years of experience or a combination of higher education and experience if a High School/GED is the highest education obtained
• At least two (2) years of experience or a combination of higher education and experience if an Associate's degree is the highest education obtained
• At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained
• Experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities
• Valid driver's license and agency established minimum automobile coverage required
