About the position

The Community Support Specialist for the Emergency Room Enhancement program plays a crucial role in engaging consumers in services and assisting them in accessing necessary resources to achieve and maintain health and recovery. This position is essential for supporting clients in navigating mental health and medical services, as well as various public services, ensuring they receive the help they need in a collaborative team environment.

Responsibilities

• Provide individual assistance to consumers in accessing needed mental health and medical services
• Assist consumers in accessing a variety of public services including financial and housing support
• Help consumers utilize community agencies for ongoing social, educational, vocational, and health resources
• Train, coach, and support consumers in daily living skills such as housekeeping, cooking, and budgeting
• Follow up with consumers regarding appointments and completion of necessary forms
• Assist clients with transportation as clinically indicated, with driving being an essential function of the position
• Perform other duties as assigned

Requirements

• High School/GED required
• At least four (4) years of experience or a combination of higher education and experience if a High School/GED is the highest education obtained
• At least two (2) years of experience or a combination of higher education and experience if an Associate's degree is the highest education obtained
• At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained
• All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities
• A valid driver's license and agency established minimum automobile coverage required

Nice-to-haves
