About the position

The Community Support Specialist role at WeCare 365 AFC involves providing social work assessments and referrals to clients and families in the Adult Foster Care program. This position serves as a liaison between clients and community-based support services, ensuring quality care through effective communication and collaboration with the multidisciplinary team. The role requires travel within Massachusetts and involves conducting on-site visits to assess and support members and caregivers.

Responsibilities

• Conducting on-site visits with the member at the qualified setting including initial Intake by member and caregiver applying to AFC program.
• Completing Community Support Specialist progress notes corresponding with each on-site visit and encounter with members.
• Providing culturally appropriate health education, information, support, and counseling to members and caregivers.

Requirements

• High School Diploma or GED
• Demonstrated leadership ability
• Excellent verbal and written communication skills
• Commitment to quality care and the multidisciplinary team approach
• CORI, Physical in the past 12 months, and Annual TB Test

Nice-to-haves

• Experience in Leadership
• Knowledge of Spanish
• Knowledge of Cape Verdean
