About the position

ReDiscover is seeking a Community Support Specialist to join our Transitional Living Program, which has been a cornerstone of our mission for over 50 years. As a non-profit community mental health center located in southeastern Jackson County, Missouri, we are dedicated to providing efficient and cost-effective treatment to individuals affected by mental illness or substance use. Our diverse team of over 700 employees is passionate about supporting our community, particularly young adults striving for independence. The Transitional Living Program is designed to empower young adults to achieve their individualized goals for independent living. Through the support of life coaches and community support specialists, participants are guided in navigating the challenges of living independently in their own one-bedroom apartments. We focus on the strengths of each young adult and work collaboratively to define what success looks like for them in the long term. Our program offers intensive daily support, case management, life coaching, medication management, and access to resources and entitlements, all within a client-centered, strengths-based supportive environment. In this role, you will maintain and interact with a caseload of clients, providing both in-person and telephonic support. You will facilitate clients' achievement of long-range outcomes related to symptom management, employment, and independent living, tailored to their unique needs. Engaging with difficult-to-serve clients will be a key aspect of your work, as you explore all potential resources to help them gain access to necessary services. You will also assist clients in overcoming barriers to participation in community programs and will be responsible for transporting clients to various appointments using your personal vehicle. Additionally, you will document client interactions in our designated electronic medical record system and collaborate with a multi-disciplinary team to ensure comprehensive care and positive communication regarding client wellness.

Responsibilities

  • Maintain and interact with a caseload of clients either in-person or over the phone.
  • Facilitate each client's achievement of long-range outcomes related to symptom management, employment, and independent living.
  • Engage difficult-to-serve clients by investigating all potential resources to gain access and make connections.
  • Help remove barriers for clients, including participation in community or social service programs and referrals to educational or working programs.
  • Assist clients with transportation barriers as identified in their treatment plan, including transporting clients in the employee's personal vehicle.
  • Collect and maintain information about clients and document interactions within ReDiscover's designated electronic medical record.
  • Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.

Requirements

  • A Bachelor's degree or four years of equivalent experience in lieu of a degree.
  • A valid automobile driver's license and appropriate automobile insurance.
  • A safe, reliable, registered automobile available for use during work hours.
  • At least two years of clinical experience in direct service provision to children and adolescents in a mental health setting (preferred).
  • Knowledge of community resources and health and social service systems in the Kansas City metro area (preferred).
  • Excellent customer service skills with a recovery focus.
  • Strong written, oral, and interpersonal communication skills.
  • Intermediate computer skills in typing/keyboarding, Microsoft Word, Excel, and GSuite.

  • Experience working with children and adolescents in a mental health setting.
  • Familiarity with community resources in the Kansas City area.
  • Strong networking skills with community partners.
