About the position

The Community Support Specialist at SHIELDS for Families is responsible for administering Housing Community Support Services to Medi-Cal beneficiaries facing homelessness and complex health and social needs in South Los Angeles. This role involves conducting outreach, maintaining a caseload of clients, and collaborating to create individualized service plans that address various personal objectives, including housing goals and income maintenance.

Responsibilities

  • Meet monthly productivity by completing required monthly visits for 28 unique clients.
  • Maintain a caseload of 35 to 40 clients to ensure the ability to meet productivity requirements.
  • Conduct outreach at various provider locations and community organizations.
  • Provide outreach and services to clients in the field, at SHIELDS locations, and telephonically.
  • Conduct intake and personalized needs assessments for all clients.
  • Collaborate to craft an Individualized Service Plan (ISP) targeting housing goals and other personal objectives.
  • Facilitate referrals and provide support to resources aiding clients in achieving their ISP objectives.
  • Complete assessments tailored to the program, such as VI-SPDAT.
  • Assess strengths and obstacles in achieving short-term and long-term goals, including conducting a 90-day review with the client post-initial intake.
  • Attend and complete all trainings as needed.
  • Participate in all staff meetings and supervision sessions.
  • Adhere to documentation standards mandated by the Program and SHIELDS policies.
  • Engage effectively with colleagues, participants, and stakeholders.
  • Contribute actively to team objectives and foster positive working relationships.
  • Convey information clearly and compellingly, both verbally and in writing.
  • Demonstrate proactive decision-making and action-taking.
  • Innovate solutions to challenges and evaluate information critically.

Requirements

  • Two years of experience in healthcare, case management, homeless services, or working with vulnerable populations.
  • High School Diploma or GED.
  • Proficient in word processing and personal computer use.
  • Strong analysis skills and judgment for decision-making.
  • Ability to present ideas and information clearly, both verbally and in writing.
  • Current California driver's license with an acceptable driving record.

Nice-to-haves
