About the position

The Community Support Specialist I (CSS I) position is a vital role within the Mental Health Services department, focusing on providing comprehensive support to clients with mental health challenges. The primary objective of this role is to assist clients in achieving a meaningful level of independence and well-being through skill-building and case management activities. The CSS I will engage in a team approach to deliver rehabilitative supports, facilitate psycho-educational groups, and promote socialization and community integration. This position requires the completion of screenings and assessments to develop person-centered care plans tailored to individual client needs. In this role, the Community Support Specialist will establish rapport and maintain professional boundaries with clients, ensuring they receive the necessary resources to secure and maintain affordable housing. Regular contact with clients is essential, as the specialist will coordinate referrals for various services, including treatment, medical care, and employment assistance. The CSS I will also enhance clients' independent living skills through various teaching modalities and assist them in identifying and overcoming barriers to their goals. The position involves conducting regular inspections of clients' living spaces, facilitating psycho-educational groups, and providing case management support to the Housing First & Pilots staff and clients. The CSS I will also be responsible for documenting client progress and participating in staff meetings to ensure a collaborative approach to client care. This role is crucial for supporting individuals with persistent chronic mental illness and/or addiction, and it encourages veterans and individuals with disabilities to apply.

Responsibilities

  • Establish rapport and positive working relationships with clients while maintaining professional ethics and boundaries.
  • Support clients' integration into the community by providing resources for securing and maintaining affordable housing.
  • Maintain regular contact with clients as defined by the care plan.
  • Coordinate and assist clients with referrals for entitlements, treatment, medical care, transportation, and other services.
  • Develop, implement, and monitor person-centered plans with clients throughout their care process.
  • Enhance clients' independent living skills through various teaching modalities.
  • Assist clients in identifying barriers to achieving their goals and link them to appropriate services.
  • Conduct regular inspections of clients' living spaces and assist in maintaining cleanliness.
  • Develop and maintain working relationships with families, natural supports, and external providers.
  • Facilitate psycho-educational groups as needed and administer breathalyzer tests and urinalysis samples when applicable.
  • Supervise and document self-administration of client medications as necessary.
  • Attend and participate in staff meetings and supervision sessions.
  • Provide additional case management support to the Housing First & Pilots staff and clients as needed.

Requirements

  • Bachelor's Degree required with a minimum of two (2) years' experience in a related behavioral health field.
  • Prior experience working with individuals with persistent chronic mental illness and/or addiction.
  • Valid Connecticut Driver's license and willingness to transport clients in personal vehicles.
  • First Aid/CPR certification or willingness to obtain certification within 90 days of hire.
  • Proficient computer skills, including Microsoft Office and electronic record keeping.
  • Knowledge of the community support system and resources preferred.
