About the position

The Community Support Specialist for the ICORT Team in McComb, MS, is responsible for providing case management services to adults with severe and persistent mental illness. This role involves working within a team-oriented approach to support individuals in achieving their recovery and resiliency goals, primarily through community-based rehabilitation and outreach services. The position requires some on-call duties and emphasizes face-to-face, phone, or video interactions with clients and their families.

Responsibilities

• Provide case management services to adult participants on the ICORT Team.
• Assist individuals in setting and attaining their recovery and resiliency goals.
• Deliver services primarily in natural settings, including face-to-face, phone, or video conference interactions.
• Support individuals in maintaining their community living and avoiding placement in state-operated behavioral health programs.
• Collaborate with team members to ensure effective rehabilitation interventions.

Requirements

• Bachelor's degree in behavioral healthcare required.
• Must have reliable transportation to provide community services.
• Must have a valid driver's license with a satisfactory driving record.
• Must be able to obtain DMH case management certification within 60 days of hire.
• Must have auto insurance liability limits of 100,000/300,000/50,000 within 1 month of hire.
• Behavioral healthcare experience preferred.
