About the position

The Community Support Specialist CSTAR/ICTS plays a vital role in providing integrated, holistic, community-based support services for the CSTAR and/or ICTS teams. This full-time position is designed to deliver quality and compassionate care to individuals facing challenges related to substance abuse and mental health. The specialist will utilize a motivational, stage-matched approach to offer individualized and group education services tailored to the strengths, needs, abilities, and preferences of clients and their families. The focus is on supporting clients in their journey toward recovery and resiliency goals, ensuring that services are delivered from a holistic perspective that incorporates the Eight Dimensions of Wellness. In this role, the specialist will assist clients in identifying their strengths and needs, participate in the development of Wellness Plans, and support clients in achieving their goals. This includes maintaining contact with clients who are hospitalized, participating in discharge planning, and collaborating with various providers to develop comprehensive aftercare plans. The specialist will also provide education and skill-building in essential areas such as daily living skills, employment, symptom management, and budgeting, with the aim of fostering independence and self-sufficiency. Additionally, the Community Support Specialist will work closely with an integrated health team, communicating actively with psychiatrists, primary care physicians, and other specialists to ensure a coordinated approach to client wellness. This includes making referrals to additional services as needed and acting as a liaison between clients and their support systems, including family members and community resources. The position requires a commitment to culturally competent service delivery and may involve assisting clients with transportation needs as clinically indicated. Overall, the role is essential in promoting recovery efforts and enhancing the quality of life for clients in the community.

Responsibilities

  • Provide individualized and group education services based on strengths, needs, abilities, and preferences identified collaboratively with clients and families.
  • Assist clients in identifying strengths, needs, abilities, and preferences regarding substance abuse and mental illness, and participate in the development of Wellness Plans.
  • Support clients in completing goals identified on Wellness Plans and review these plans as needed for updates.
  • Aid clients in accessing public services, including financial, housing, emergency assistance, and food.
  • Maintain contact with hospitalized clients, participate in discharge planning, and communicate with providers to develop aftercare plans.
  • Provide education and skill-building in areas such as daily living skills, employment, symptom management, and budgeting.
  • Assist clients in developing personal support systems, including work with family members and recovery communities.
  • Provide individual assistance to clients in accessing needed services, including accompanying them to appointments.
  • Follow up with clients regarding appointments and completion of necessary forms.
  • Contact consumers with unexcused absences to re-engage them in recovery efforts.
  • Meet service provision expectations per client needs and company guidelines.
  • Complete documentation and paperwork within required timeframes.
  • Collect and process urine drug screens.
  • Communicate actively with integrated health team members regarding client wellness, medication adherence, and side effects.
  • Seek input from integrated health team providers on clients' Wellness Plans.
  • Make referrals to additional services/providers as needed for clients' integrated health needs.
  • Function as a liaison to others involved in clients' treatment, including family, employers, and legal entities.

Requirements

  • High School diploma or GED required; Associate's or bachelor's degree in a human service-related field preferred.
  • At least four years of experience in a relevant field if no degree is obtained, or a combination of higher education and experience.
  • At least two years of experience required if an Associate's degree is the highest education obtained.
  • At least one year of experience required if a non-human service bachelor's degree is the highest education obtained.
  • All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities.
  • A valid driver's license and agency-established minimum automobile coverage are required.
