About the position

Hennepin Home Health Care is expanding their Individualized Home Supports (IHS) department and is seeking qualified Community Support Specialists in the Twin Cities to join our team. This role is designed for individuals who are passionate about providing mental health services to community members, helping them maintain independence and thrive. As a Community Support Specialist, you will focus on delivering mental health support, which includes managing symptoms of mental illness, utilizing and integrating community resources, providing crisis assistance, and implementing relapse prevention strategies. You will also assist clients with household management and promote healthy lifestyle skills and practices. In this position, you will work closely with clients to develop personalized support plans that address their unique needs and challenges. You will be responsible for conducting assessments, monitoring progress, and adjusting support strategies as necessary. The role requires a strong commitment to fostering a supportive and empowering environment for clients, ensuring they have access to the resources they need to succeed. You will also collaborate with other healthcare professionals and community organizations to enhance the support network available to clients. Hennepin Home Health Care prides itself on being a supportive company that values the growth and development of its employees. We offer flexible schedules to promote a healthy work/life balance, and our working hours are Monday through Friday from 8 AM to 5 PM. Employees will be provided with a company-issued iPad to access community resources and complete required documentation. Additionally, travel costs will be covered upfront through a company gas card, allowing you to focus on your work without financial concerns. Our management team is dedicated to your success and will provide education and training on relevant topics to help you enhance your skillset.

Responsibilities

  • Provide mental health support including symptom management and crisis assistance.

Requirements

  • Bachelor's degree in a relevant field.
  • Minimum of 2 years of experience in human services or related field.
  • Strong understanding of mental health issues and support strategies.
  • Ability to travel up to 50% of the time.

Nice-to-haves

  • Experience in addiction counseling or caregiving.
  • Familiarity with community resource networks.
