About the position

The Community Support Specialist II role at BJC Behavioral Health focuses on providing community support services to individuals recovering from serious mental illness. This position aims to maximize opportunities for clients by utilizing treatment plan interventions tailored to individual strengths and needs, ultimately promoting recovery and resilience within the community.

Responsibilities

• Provides community support services to complex clients, including those on care plans, using treatment plan interventions that yield positive outcomes.
• Completes timely documentation of services, detailing the need for the service, the intervention provided, and the client's response.
• Contacts individuals and referral sources after missed appointments to re-engage clients and support recovery efforts.
• Supports individuals in crisis situations and provides mentorship and job shadowing to community support colleagues.
• Assists clinical teams with qualified mental health provider support, including assessments and access to services.
• Completes annual assessments and other necessary documentation for clients.

Requirements

• Master's Degree in Human Services or a related field.
• 2-5 years of relevant experience.
• Licenses & Certifications: Class D (IL) or Class E (MO).
