About the position

The Community Support Specialist II position at Community Health Resources (CHR) is a full-time role focused on providing essential support to individuals with serious mental illnesses. This position is part of the Community Support Program (CSP) located in Manchester, CT, which is designed to assist clients in living independently through a community-based rehabilitation model. The role involves working closely with clients to help them develop the skills necessary for independent living, access community resources, and achieve their personal recovery goals. Community Support Specialists serve as advocates for their clients, ensuring they receive appropriate clinical services, housing benefits, and support for their human rights. In this role, you will conduct clinical and risk assessments, including LOCUS and co-occurring screens, and collaborate with clients to create individualized, person-centered treatment plans. You will document client progress and utilize motivational interventions to assist clients in meeting their goals. The position also involves providing psycho-educational rehabilitation services, both in group and individual settings, to support recovery from psychiatric and substance abuse disorders. You will help clients link with natural supports, develop crisis plans, and provide skills teaching in various areas such as housing maintenance, transportation, nutrition, and self-advocacy. The Community Support Specialist II will participate in family education and support activities, attend staff meetings, and may perform other duties as needed. This position requires a Bachelor's degree in a related field and at least two years of experience working with individuals who have mental health or substance abuse issues. A valid driver's license is required, and certification in recovery support is recommended for those who identify as having a mental illness or substance abuse background. Responsibilities

  • Conduct clinical and risk assessments, including LOCUS and co-occurring screens.
  • Collaborate with individuals to create individualized, person-centered treatment plans and document client progress during treatment.
  • Use motivational interventions to assist clients in meeting their goals and moving along their paths to recovery.
  • Provide psycho-educational rehabilitation services in group and individual settings for recovery from psychiatric and substance abuse disorders.
  • Support individuals in linking and engaging with natural (family, friends) or community supports.
  • Develop crisis plans for clients and work with other staff to respond to crisis situations as needed.
  • Provide skills teaching, such as assisting clients in obtaining/maintaining housing and utilities, learning how to use transportation services, and planning nutritional meals.
  • Engage and communicate with family and friends, ensure safety in the community, and solve everyday problems that arise.
  • Provide intensive skill-building interventions and support for self-management, self-advocacy, health and well-being, and relapse prevention.
  • Participate in family education and support activities.
  • Document client progress in Electronic Medical Record per agency policies.
  • Attend and participate in staff meetings, trainings, case conferences, supervision, and other meetings as assigned.

Requirements

  • Bachelor's Degree in a related field.
  • 2+ years of experience working with identified populations, particularly those with mental illness or substance abuse issues.
  • Valid driver's license required.
  • Recovery Support Certification recommended for individuals who identify as having a mental illness or substance abuse.
