About the position

The Community Support Specialist II role at Community Health Resources (CHR) focuses on providing mental health and substance use rehabilitation services to adults. The position emphasizes individualized support, skill-building, and promoting independence among clients. Specialists work closely with clients to develop personalized treatment plans and assist them in achieving their recovery goals, all while enjoying a supportive and flexible work environment.

Responsibilities

  • Partnering with individuals to accomplish their recovery goals.
  • Conducting Co-occurring Screens, C-SSRS, and Functional Assessments.
  • Collaborating with individuals to create individualized, person-centered treatment plans and documenting client progress during treatment.
  • Using Motivational Interventions to assist clients in meeting their goals and progressing in their recovery paths.
  • Providing psycho-educational rehabilitation services via group and individual modalities.
  • Supporting individuals in linking and engaging with natural (family, friends) or community supports.
  • Developing crisis plans and Safety Plans for clients and responding to crisis situations as needed.
  • Providing skills teaching, such as assisting clients in obtaining/maintaining housing and utilities, using transportation services, planning and preparing nutritional meals, and solving everyday problems.
  • Participating in family education and support activities.
  • Documenting client progress per agency policies.
  • Attending and participating in staff meetings, trainings, case conferences, supervision, and other assigned meetings.

Requirements

  • Bachelor's degree or equivalent education and/or work experience required.
  • 1+ years working with identified population preferred.
  • Valid driver's license required.

Nice-to-haves

  • Experience in mental health or substance use rehabilitation services.
  • Familiarity with motivational interviewing techniques.
