About the position

The Community Support Specialist II plays a crucial role in providing services to individuals with Psychiatric Disabilities who are living independently in the community. This position operates under the guidance of the Program Supervisor and collaborates with other team members to implement a person-centered approach. The primary focus is on planning and facilitating recovery-oriented rehabilitation and enrichment services that empower individuals to achieve their personal mental health recovery goals and lead fulfilling lives. The Community Support Specialist II is responsible for ensuring that the daily operations within the workgroup align with Transitional Services, Inc.'s (TSI) mission statement and guiding principles, as well as the agency's Psychiatric Rehabilitation Approach Mission statement and values. In this role, the Community Support Specialist II will engage with individuals served, helping them to actively participate in their recovery journey. This includes developing personalized plans that cater to their unique needs and aspirations. The specialist will also be involved in coordinating various services and resources that support the individuals in their recovery process. The position requires strong organizational skills and the ability to manage time effectively, as the specialist will be responsible for maintaining accurate records and documentation related to the services provided. The work schedule for this position is Monday through Friday, from 8 am to 4 pm, providing a structured work environment that allows for consistent support to the individuals served. Additionally, a sign-on bonus of $1,000 is offered to new hires, reflecting the organization's commitment to attracting qualified candidates who are passionate about making a difference in the lives of those with psychiatric disabilities.

Responsibilities

  • Provide services to individuals with Psychiatric Disabilities living in their own housing.

Requirements

  • High school diploma or GED required.
  • Bachelor's degree in a health-related field preferred.
  • One year of experience in Psychiatric Disabilities, Mental Health, Intellectual Disabilities, Residential, or Human Services.
  • Strong time management and organizational skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Excellent communication skills.
  • Basic computer data entry skills required.
  • Valid PA Driver's License.
