About the position

The Community Support Specialist II/Mental Health Case Manager/Assessment Writer position at New Horizons Community Support Services, Inc. involves providing case management, rehabilitation services, and assessment services to adults with serious mental illness. The role requires a commitment to assisting consumers in achieving their goals, improving their coping skills, and enhancing their overall wellness through various interventions and support systems. This full-time position includes participation in an on-call rotation and requires training in Crisis Prevention Institute (CPI).

Responsibilities

• Provide interventions to assist consumers with identifying and meeting their goals, learning new skills, and improving their coping and symptom management skills.
• Assist consumers in developing interpersonal, family, and community relationships, as well as independent living skills.
• Ensure follow-through with recommended health care, including scheduling appointments and arranging transportation for consumers.
• Provide clinical expertise to complete screenings, psychosocial assessments, and crisis prevention planning/interventions.
• Develop, implement, and update treatment plans that identify specific, individualized, and measurable wellness and recovery goals.
• Clearly document clinical interventions in progress notes.
• Implement and abide by company policy.

Requirements

• Must be a Qualified Mental Health Professional (QMHP) with a master's degree in a relevant field such as counseling, psychology, or social work.
• Experience in outpatient mental health is a plus.
• Must have completed a practicum or have one year of experience under the supervision of a QMHP.

Nice-to-haves
