About the position

The Community Support Specialist II position is a full-time role focused on providing community support services to individuals with severe mental health challenges. The specialist will work directly in the community, assisting clients in accessing resources such as housing and food assistance, while maintaining a caseload that may vary. This role emphasizes face-to-face interactions with clients and requires local travel using a personal vehicle.

Responsibilities

• Provides community support services to complex clients, utilizing treatment plan interventions for positive outcomes.
• Completes timely documentation of services, detailing the need for service, interventions provided, and client responses.
• Contacts individuals or referral sources after missed appointments to re-engage clients and support recovery efforts.
• Provides mentorship and job shadowing to community support colleagues.

Requirements

• Bachelor's Degree in Human Services or a related field.
• Valid Class D (IL) or Class E (MO) driver's license.
