Responsibilities

• Assist clients by linking them to community resources such as housing and food assistance.
• Provide community support services to complex clients, utilizing treatment plan interventions for positive outcomes.
• Complete timely documentation of services, detailing the need for service, interventions provided, and client responses.
• Contact individuals or referral sources following missed appointments to re-engage clients.
• Support individuals in crisis situations.
• Provide mentorship and job shadowing to community support colleagues.

Requirements

• Bachelor's Degree in Human Services or a related field.
• Valid Class D (IL) or Class E (MO) driver's license.

Nice-to-haves

• Experience with care plans.
• Related work experience with individuals suffering from mental health challenges.
