About the position

The Community Support Specialist II plays a crucial role in providing mental health and substance abuse rehabilitation services to individuals, helping them achieve and maintain independence. This position involves working closely with clients to develop personalized care plans, facilitate psycho-educational groups, and ensure a supportive environment for recovery. The specialist will engage in various activities, including crisis intervention, skill-building, and coordination of services, all aimed at enhancing the clients' quality of life.

Responsibilities

  • Establish rapport and positive working relationships with clients while maintaining professional ethics and boundaries.
  • Enhance clients' independent living skills through various teaching modalities.
  • Coordinate and assist clients with referrals and maintaining entitlements, treatment, and medical care.
  • Develop, implement, and monitor person-centered plans with clients.
  • Assist clients in identifying barriers to their goals and link them to appropriate services.
  • Conduct regular meetings with clients to review progress and discharge planning.
  • Perform apartment/room inspections and assist clients in maintaining cleanliness.
  • Develop and maintain relationships with families and external providers.
  • Facilitate psycho-educational groups as needed.
  • Administer breathalyzer tests and urinalysis samples as applicable.
  • Supervise and document self-administration of client medications as applicable.
  • Maintain a clean and organized work environment and ensure workplace safety.
  • Participate in fire and emergency drills and maintain emergency preparedness plans.
  • Adhere to policies and practices outlined in the staff handbook and program operations manual.
  • Maintain accurate electronic records and case files in accordance with requirements.

Requirements

  • Bachelor's Degree required with a minimum of two years' experience in a related behavioral health field.
  • Prior experience working with individuals with chronic mental illness and/or addiction.
  • Valid Connecticut Driver's license and reliable transportation.
  • First Aid/CPR certification or willingness to obtain within 90 days of hire.
  • Proficient computer skills, including Microsoft Office and electronic record keeping.

Nice-to-haves

  • Knowledge of the community support system and resources preferred.
  • Experience in Trauma Informed Care.
