About the position

The Community Support Specialist III is responsible for providing community support services to adults recovering from serious mental illness. This role involves advocating for client needs, completing treatment plans, and ensuring timely documentation of services. The specialist will work closely with clients in various settings, including their homes, to promote recovery and resilience.

Responsibilities

• Provides community support services to complex clients, utilizing treatment plan interventions for positive outcomes.
• Completes timely documentation of services, detailing the need for service, intervention provided, and client response.
• Contacts individuals or referral sources after missed appointments to re-engage clients and support recovery efforts.
• Mentors and provides job shadowing to community support colleagues.
• Assists clinical teams with qualified mental health provider support, including service access assessments.
• Completes annual assessments and other necessary documentation for clients.

Requirements

• Master's Degree in Human Services or a related field.
• 2-5 years of relevant experience.
• Licenses: Class D (IL) or Class E (MO).
