About the position

The Community Support Specialist IV at Legacy Treatment Services is responsible for developing, implementing, and monitoring individualized habilitation plans for consumers. This role focuses on therapeutic rehabilitative skill development, crisis intervention, and coordinating mental and physical health services to support community integration and restoration of functioning.

Responsibilities

• Contribute to the development and implementation of individualized habilitation plans.
• Monitor and update consumers' habilitation plans as needed.
• Implement therapeutic rehabilitative skill development and Illness Management and Recovery (IMR) principles.
• Coordinate and manage mental health and physical health services.
• Provide crisis intervention as necessary.
• Maintain own appointment schedule and meet work productivity requirements.
• Perform other duties as assigned by management staff.

Requirements

• Bachelor's degree from an accredited college or university in a helping profession (social work, counseling, psychiatric rehabilitation, or psychology).
• Minimum of 1 year of professional experience in a community-based behavioral health setting.
• If the degree is in a field other than a helping profession, a minimum of 2 years of professional experience is required.
• Certified Psychiatric Rehabilitation Practitioner (CPRP) may substitute for one year of experience.
