About the position

This position is focused on the development and implementation of individualized habilitation plans for consumers, emphasizing therapeutic rehabilitative skill development and the application of Illness Management and Recovery (IMR) principles. The role involves coordinating mental and physical health services, providing crisis intervention, and ensuring community integration for consumers. The position requires maintaining a schedule and meeting productivity requirements while also operating an Agency vehicle.

Responsibilities

• Contribute to the development and implementation of individualized habilitation plans for consumers.
• Facilitate therapeutic rehabilitative skill development.
• Implement Illness Management and Recovery (IMR) principles and interventions.
• Coordinate and manage mental health and physical health services.
• Provide crisis intervention as needed.
• Ensure active rehabilitation and support services for community integration.
• Maintain own appointment schedule and meet work productivity requirements.
• Perform other duties as assigned by management staff.

Requirements

• Bachelor's Degree from an accredited college or university in a helping profession (social work, counseling, psychiatric rehabilitation, or psychology).
• If the degree is in a field other than a helping profession, a minimum of two years' professional experience in a community-based behavioral health setting is required.
• Certified Psychiatric Rehabilitation Practitioner (CPRP) may substitute for one year of experience.
