About the position

The Community Support Specialist is responsible for providing community-based healthcare services for individuals dealing with substance abuse, mental health issues, and co-occurring disorders. The role focuses on maximizing client independence and responsibility while promoting recovery and resilience through personalized care plans and support systems. The specialist works closely with clients to facilitate their adjustment and functioning within the community, ensuring they receive the necessary services to achieve sobriety and sustained recovery.

Responsibilities

  • Providing holistic, person-centered care emphasizing individual strengths and skills acquisition.
  • Updating treatment plans continually based on client records.
  • Facilitating recovery/resiliency through goal setting and resource identification.
  • Developing and revising care plans with measurable interventions.
  • Maintaining contacts with referral and third-party payer sources as required.
  • Documenting services provided, including client responses and service details.
  • Creating discharge and aftercare plans for continued recovery support.
  • Educating clients and families about managing mental illness and substance use disorders.
  • Building skills for effective illness self-management and coping strategies.
  • Transporting clients to services and therapeutic activities as needed.

Requirements

  • Bachelor's Degree in social work, psychology, nursing, or a closely related field, or 3 years of relevant experience.
  • Experience providing one-on-one or group services focused on rehabilitation and recovery.
  • Knowledge of substance use disorders and co-occurring mental health diagnoses.
  • Excellent written and verbal communication skills.
  • Ability to develop and maintain organized records and prepare reports.
  • Experience working with diverse cultures and backgrounds.
