About the position

The Community Support Specialist (CSS) at Canopy Children's Solutions plays a crucial role in providing tailored care services to children and their families. This position focuses on assessing individual needs and linking families with essential community resources, ensuring they can access educational, medical, social, and recreational services. The CSS is dedicated to fostering community integration and support for families, reflecting the organization's core values of prioritizing the voices of children and families, building strong relationships, and delivering exceptional service.

Responsibilities

  • Assist children and families to gain access to needed community support services including education, medical, social, vocational, recreational, therapeutic, and other social services.
  • Assess the strengths and service needs of the children and families.
  • Develop specific, measurable, individualized interventions and services to meet needs and measure progress.
  • Provide transportation for families in a safe and responsible manner.
  • Direct services in the home, school, and community whenever possible.
  • Provide and coordinate social services that are child-focused and build on family and community strengths.
  • Provide crisis intervention services as needed within the scope of role and competency.
  • Develop and maintain a comprehensive, up-to-date community support resource manual.
  • Implement service interventions based on the ISP and document in Evolv.
  • Complete necessary documentation for each child and family based on program guidelines.
  • Demonstrate flexibility and work with the family based on their schedule, which may require working some evenings and weekends.
  • Attend weekly staff meetings.
  • Collaborate with supervisor, therapist, and other MCHS staff to monitor progress.
  • Assist with aftercare planning to ensure referrals and appointments are made, and services are available/accessible.

Requirements

  • Minimum of Bachelor's degree in social work or related field.
  • At least two years of experience working directly with children and families.
  • Must meet the minimum requirements to be certified through the Mississippi Department of Mental Health as a Community Support Specialist.
  • Demonstrate competence implementing evidence-based practices and best practices working with children and families.
  • Competence in completing reports related to payment for services provided.
  • Computer skills required.
  • Must be at least 23 years of age.
  • Valid Mississippi driver license and a good driving record.
  • Proof of legally appropriate auto liability insurance.
  • Willingness to use personal vehicle for agency business.
