About the position

The Community Support Specialist (CSS) plays a crucial role in providing care services tailored to the individual needs of children and their families. This position involves delivering a range of community services, assessing needs, and linking families with social, educational, vocational, and recreational resources. The CSS works collaboratively with families to ensure they can effectively integrate into their communities and access necessary services, reflecting the core values of Canopy Children's Solutions.

Responsibilities

  • Assist children and families in accessing community support services such as education, medical, social, vocational, and recreational services.
  • Assess the strengths and service needs of children and families.
  • Develop specific, measurable, individualized interventions and services to meet needs and track progress.
  • Provide safe and responsible transportation for families.
  • Direct services in home, school, and community settings whenever possible.
  • Coordinate child-focused social services that build on family and community strengths.
  • Provide crisis intervention services as needed within the scope of the role.
  • Maintain a comprehensive, up-to-date community support resource manual.
  • Implement service interventions based on the Individual Service Plan (ISP) and document in Evolv.
  • Complete necessary documentation for each child and family based on program guidelines.
  • Demonstrate flexibility in working with families based on their schedules, including evenings and weekends.
  • Attend weekly staff meetings and collaborate with supervisors and other staff to monitor progress.
  • Assist with aftercare planning to ensure referrals and appointments are made.

Requirements

  • Minimum of Bachelor's degree in social work or related field.
  • At least two years of experience working directly with children and families.
  • Must meet the minimum requirements to be certified through the Mississippi Department of Mental Health as a Community Support Specialist.
  • Demonstrate competence in implementing evidence-based practices and best practices for working with children and families.
  • Competence in completing reports related to payment for services provided.
  • Computer skills required.
  • Must be at least 23 years of age and have a valid Mississippi driver license with a good driving record.
