About the position

ReDiscover is seeking a Community Support Specialist to join our Opioid Treatment Department. With over 50 years of experience as a non-profit community mental health center in southeastern Jackson County, Missouri, ReDiscover is dedicated to delivering efficient and cost-effective treatment for individuals affected by mental illness or substance use. Our diverse team of over 700 employees is passionate about supporting our community, particularly those impacted by opioid use disorder. The Community Support Specialist will play a crucial role in our Opioid Treatment Programs, which provide medication-assisted treatment options such as methadone and buprenorphine, alongside counseling, group therapy, case management, and nursing services. The primary goal of this position is to prevent overdose, alleviate withdrawal symptoms, normalize brain chemistry, and help clients achieve their personal life goals. In this role, you will maintain and interact with a caseload of clients, facilitating their journey towards symptom management, employment, and independent living. You will engage with clients who may be difficult to serve, exploring all potential resources to help them access necessary services. Additionally, you will assist clients in overcoming barriers to their treatment plans, which may include connecting them with community programs, educational opportunities, or transportation assistance. Documentation of client interactions and collaboration with a multi-disciplinary team will be essential to ensure comprehensive care and positive outcomes for our clients.

Responsibilities

  • Maintain and interact with a caseload of clients with diverse characteristics either in-person or over the phone.
  • Facilitate each client's achievement of long-range outcomes of symptom management, employment, and independent living based on their needs.
  • Engage difficult-to-serve clients by investigating all potential resources to gain access and make connections.
  • Help remove barriers for clients, including participation in community or social service programs, referrals to educational or working programs, and assistance with transportation barriers as identified in their treatment plan.
  • Collect and maintain information about clients and document interactions within ReDiscover's designated electronic medical record.
  • Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.

Requirements

  • Bachelor's degree in a social science field or 4 years of equivalent experience in lieu of degree.
  • Valid automobile driver's license and appropriate automobile insurance.
  • Working knowledge of mental illness and substance use disorders.
  • Excellent customer service skills with a recovery focus.
  • Strong written, oral, and interpersonal communication skills.
  • Intermediate computer skills in typing/keyboarding, Microsoft Word, Excel, and G-Suite.
