The Community Support Specialist role at Thresholds is a part-time position focused on supporting clients in their mental health recovery through case management and care planning. The specialist will assist clients in navigating healthcare systems, strengthen their daily living skills, and provide transportation to various appointments. Responsibilities

• Assist clients in navigating mental healthcare systems and accessing necessary resources.
• Provide transportation to clients for appointments, benefits offices, and grocery shopping.
• Document services provided in the Electronic Health Care Record.
• Support clients in strengthening their daily living skills.
• Adapt to clients' schedules, needs, and treatment plans.

Requirements

• Passion for mental health advocacy.
• Effective communication and relationship-building skills.
• Good writing skills for documentation purposes.
• Strong time management and organizational skills.
• Must be at least 21 years old.
• Valid driver's license, current car insurance, and daily access to a car.

Nice-to-haves

• High school diploma or GED with 5 years of supervised clinical experience.
• Bachelor's degree in Psychology, Social Work, or a related field.
• Bachelor's degree in an unrelated field with 2 years of supervised clinical experience.
• Master's degree in Psychology, Social Work, or a related field.
