About the position

Fresh Start Therapeutic Services, LLC is a qualified wrap facility and mental health clinic that provides intensive outpatient mental and behavioral health services to clients who are at risk of homelessness, hospitalization, school expulsion, or who cannot fully function in all life domains. We are currently seeking Community Support Service Providers (CSSP) to join our dedicated team. The primary role of the CSSP is to assist clients in developing life skills, serving as the main point of contact for clients, and working closely with them to enhance their capabilities and overall well-being. The CSSP will typically work with a single client for 3-6 days a week, providing site-based services in various locations such as the client's home, school, or adult daycare facilities. The CSSP will be responsible for delivering skills-building services that focus on increasing the functionality and assets of children, adolescents, and their communities. This role requires a commitment to assisting clients in their recovery journey, helping them improve their mental health and wellness, and strive for independence. The CSSP will also be responsible for reviewing treatment plans with clients and their legally authorized representatives (LARs), ensuring that recovery goals and interventions are clearly defined and updated as necessary throughout the treatment process. Documentation is a critical aspect of this role, as the CSSP must maintain accurate records in Theranest, including progress notes and contact logs. Attendance at weekly staffing meetings and treatment plan reviews is also required to collaborate with the treatment team and ensure the best outcomes for clients. This position is a contract role, offering flexibility in scheduling and the potential for advancement for those with higher education qualifications.

Responsibilities

  • Provide skills-building service delivery to clients that identify, enhance capabilities, knowledge, skills, and assets of the client and their community.
  • Assist clients in their recovery process to improve mental health and wellness, live independently, and reach their full potential.
  • Deliver site-based intensive services at locations such as clients' homes, schools, or adult daycare facilities.
  • Review treatment plans with clients and LARs, specifying recovery goals, objectives, and interventions.
  • Update the Theranest calendar daily to avoid scheduling conflicts with other clients or case managers.
  • Document progress notes in Theranest following the treatment protocol, ensuring professionalism in grammar and punctuation.
  • Document all correspondence with clients and LARs in Theranest under the Contact Log in a timely manner.
  • Attend Friday staffing meetings and treatment plan reviews with the treatment team.

Requirements

  • Must have 2 years of direct mental health care experience.
  • Must have an associate degree in social services or higher education.
  • Must have reliable transportation and a valid driver's license.
  • Must have current car insurance.
  • Must undergo a background check in accordance with local laws and regulations.
  • Must submit a Tuberculosis Skin Test with a 'Negative' reading upon offer.
  • Must submit First Aid/CPR certification upon offer.
  • Must obtain an ANSA and CANS Certificate upon offer.

Nice-to-haves

  • Bilingual in English and another language is a plus.
  • Experience with medical documentation is preferred.
