About the position

The Community Support Specialist (PSH) plays a crucial role in the Permanent Supportive Housing program, which aims to connect adults with severe and complex disabilities to affordable housing and supportive services. This position focuses on providing Community Psychiatric Supports and Treatment (CPST) and Psychosocial Rehabilitation (PSR) services to help participants achieve their self-identified goals, promoting recovery and independence within the community.

Responsibilities

• Provide case management and supportive services including social skills, life skills, and job search assistance.
• Participate in setting case plan goals and objectives with clients.
• Complete case plans and maintain accurate case logs and documentation.
• Ensure clinical documentation and client files meet applicable standards.
• Communicate effectively and establish professional relationships with clients.
• Promote recovery, independence, and personal choice for participants.
• Assist clients in identifying strengths, needs, and realistic goals for service plans.
• Teach social skills and household management skills effectively.
• Demonstrate knowledge of behavioral health symptoms and interventions.
• Maintain professional boundaries and adhere to ethical guidelines.

Requirements

• Bachelor's or Master's Degree in Sociology, Psychology, Social Work, or Counseling.
• Must be over 18 years of age.
• Possess a valid and clear driver's license.
