About the position

The Community Support Specialist QMHP Youth at Compass Health Network is a vital role designed to provide integrated behavioral health community support services to children and their families. This position is essential in helping clients achieve their recovery and resiliency goals through a collaborative and strengths-based approach. The specialist will engage both the child and their parent or caregiver, working closely with them to identify their strengths, needs, abilities, and preferences. This role requires a commitment to providing services that are holistic and person-centered, focusing on the Eight Dimensions of Wellness, which encompass physical and mental health, among other areas. In this full-time position, the Community Support Specialist will be responsible for developing and implementing Wellness Plans tailored to each client’s unique situation. This includes maintaining contact with clients during hospitalizations, assisting families in accessing necessary resources, and providing educational information to improve daily living skills and symptom management. The specialist will also be involved in crisis intervention planning and will work to develop natural support systems for clients, collaborating with family members, school staff, and other stakeholders. The role requires active communication with an integrated health team, which includes psychiatrists, therapists, and primary care providers, to ensure comprehensive care for clients. The specialist will triage clients for services, make referrals, and act as a liaison between clients and their treatment providers. Additionally, the position involves completing all required documentation and assessments in a timely manner, ensuring compliance with agency policies and guidelines. This position is not only about providing clinical services but also about advocating for the mental health needs of children and families within the community. The Community Support Specialist will seek culturally competent services to meet the diverse needs of clients and will assist with transportation as clinically indicated, making driving an essential function of the role.

Responsibilities

  • Provide individualized services based on strengths, needs, abilities, and preferences identified collaboratively with client/family.
  • Assist client/family in identifying strengths, needs, abilities, and preferences regarding mental illness/substance use and participate in the development of Wellness Plans.
  • Provide services from a holistic, person-centered, strengths-based perspective, incorporating coaching surrounding the Eight Dimensions of Wellness.
  • Support client/family in the completion of goals identified on Wellness Plans, reviewing and reassessing as needed.
  • Assist the family with accessing, locating, and arranging services and resources to achieve their goals.
  • Maintain contact with client/family during hospitalization and participate in discharge planning.
  • Provide educational information and skill building regarding daily living skills, education, symptom management, and communication/relationships.
  • Develop and implement a crisis intervention plan with the family and provide crisis intervention services as needed.
  • Assist client/family in developing natural support systems, working with family members, school staff, and legal guardians.
  • Participate in and coordinate with the child and family interagency team members to advocate for mental health needs.
  • Complete all required documentation within the required timeframe according to agency policy.
  • Meet service provision expectations per client needs and agency guidelines.
  • Complete initial and annual assessments and Wellness Plans as directed by Supervisor.
  • Provide clinical input and support to the team from a QMHP perspective.

Requirements

  • Master's degree in psychology, social work, rehabilitation/vocational/pastoral counseling, family therapy, or closely related field required.
  • Completion of a practicum or at least one year of experience under the supervision of a QMHP required.
  • Experience must include working with individuals with psychiatric and substance use disorders.
  • A valid driver's license and agency established minimum automobile coverage required.
  • Qualified Mental Health Professional (QMHP) certification required.
