The Community Support Specialist plays a crucial role in providing direct services to clients within a residential mental health setting. This position involves individual and group counseling, vocational support, crisis intervention, and psycho-social rehabilitation. The specialist is responsible for identifying risk factors that may lead to relapse and developing effective relapse prevention plans and strategies tailored to each client's needs. The role requires a compassionate approach to support clients in their journey towards recovery and independence. In addition to counseling, the Community Support Specialist is tasked with developing and reviewing care plans for members. This includes identifying specific goals, objectives, and the mental health services required to meet those needs. Collaboration with collateral social service agencies and community resources is a key aspect of this role, ensuring that clients receive comprehensive support. Documentation is a critical component of the position, as the specialist must accurately record services provided in an Electronic Health Record system in a timely manner. This ensures continuity of care and compliance with organizational standards. Candidates for this position must be at least 21 years old and possess either a high school diploma or GED with five years of relevant experience, or a Bachelor's degree in Psychology, Social Work, or a related field.
