About the position

The Community Support Specialist (CSS) at Thresholds plays a vital role in providing mental health and substance use treatment support to individuals experiencing unsheltered homelessness and severe mental illness. This position focuses on care planning and case management, helping clients navigate complex systems and access necessary resources. The CSS will assist clients in strengthening their daily living skills and adapting to their unique needs, all while working within a collaborative and supportive culture that emphasizes professional development and training.

Responsibilities

  • Provide mental health and substance use treatment support in the community.
  • Assist clients in care planning and case management.
  • Connect clients with resources and help them navigate complex systems.
  • Support clients in strengthening daily living skills.
  • Transport clients to appointments, benefits offices, and grocery stores.

Requirements

  • Passion for mental health advocacy.
  • Effective communication and relationship-building skills.
  • Good writing skills.
  • Strong time management and organization skills.
  • Must be at least 21 years old.
  • Valid driver's license, current car insurance, and daily access to a car.

Nice-to-haves

  • High school diploma or GED certificate with 5 years of supervised clinical experience.
  • Bachelor's degree in Psychology, Social Work, or related field.
  • Bachelor's degree in an unrelated field with 2 years of supervised clinical experience.
  • Master's degree in Psychology, Social Work, or related field.
