About the position

The School-Based Community Support Specialist at Compass Health Network plays a crucial role in bridging the gap between children, families, schools, and external systems. This position is designed to provide a comprehensive range of services tailored to the unique needs of each child and family, ensuring that they receive the support necessary for their mental health and overall well-being. The specialist will engage in collaborative documentation with families and will be required to work evenings and weekends as needed to accommodate the schedules of the families served. Initially, the role will focus on community-based training, transitioning to school-based functions once the necessary clinical and professional competencies are demonstrated. The specialist will meet regularly with caregivers in their homes, ensuring that the care provided is consistent and effective. This full-time position operates primarily during weekdays, with some evening and weekend hours required to meet the needs of clients. The Community Support Specialist will act as an ambassador for Compass Health Network, providing mental health consultation to school staff and collaborating with multidisciplinary teams to advocate for the mental health needs of children and families. The role emphasizes a holistic, person-centered approach, focusing on the strengths and preferences of clients and their families, and assisting them in developing wellness plans and accessing necessary resources.

Responsibilities

  • Act as a liaison and ambassador for Compass Health Network to and between the agency and School Districts
  • Serve as a consultant to school systems' staff, providing mental health consultation and information on a wide range of mental health topics
  • Collaborate with other systems' staff regarding assigned children and their families, participating as a member of the multi-disciplinary team to advocate for the mental health needs of the child/family
  • Provide individualized behavioral health services based upon strengths, needs, abilities and preferences identified collaboratively with client/family, focused on supporting client's movement toward recovery/resiliency goals
  • Assist client/family in the development of the Wellness Plan
  • Provide services from a holistic, person-centered, strengths-based perspective, incorporating coaching surrounding the Eight Dimensions of Wellness; supporting client/family in completion of goals
  • Assist the family with accessing, locating and arranging services and resources to assist in achieving their goals
  • Provide educational information and skill building to client/family and school staff regarding areas of concern such as daily living skills, education, symptom management, communication/relationships with focus on improving functioning
  • Assist client/family in developing natural support systems including work with family members, school staff, legal guardians, etc.
  • Assist with completion of initial and annual assessments and Wellness Plans as directed by Supervisor
  • Seek culturally competent services and service providers to meet the unique cultural needs of each agency client and family
  • Driving is an essential function of the position

Requirements

  • High School/GED required
  • Associate's/Bachelor's/Master's degree in a human service-related field preferred
  • At least four (4) years of experience OR a combination of higher education and experience required if a High School/GED is the highest education obtained
  • At least two (2) years of experience OR a combination of higher education and experience required if an Associate's degree is the highest education obtained
  • At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained
  • All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities
  • A valid driver's license and agency established minimum automobile coverage required
