About the position

The Community Support Specialist at BHcare Inc. plays a vital role in supporting the goals of the organization by providing community support services that focus on developing individualized goals with consumers. The primary aim is to enhance the success and overall quality of life for individuals by offering skill-building and motivational support in areas such as employment, education, community involvement, and the utilization of natural supports. This position is accountable for delivering individual and group sessions while meeting specific caseload and productivity requirements. Upon admission, the specialist will conduct a functional assessment, which will be updated quarterly or every six months, depending on the consumer's needs. A person-centered recovery plan will be developed, featuring stage-wise treatment goals and objectives that are both measurable and attainable, with mandatory reviews at least every three months. The specialist will observe, record, and evaluate the consumer's progress towards their goals, documenting interventions according to the psychiatric rehabilitation skill-building model. In addition to providing outreach and crisis intervention to community support service consumers, the specialist will facilitate individual and group sessions in the community, focusing on appropriate stage-wise interventions. This includes assisting with activities of daily living (ADLs), teaching recovery skills, developing self-advocacy skills, and providing health and wellness education. Limited transportation may be provided as clients learn individual transportation skills, and assistance will be given in connecting clients to medical and psychiatric appointments as needed. The role also involves educating and supporting family members and community members to meet the rehabilitation goals set by the consumer. The specialist will develop individualized and generalized community supports, consult with the Program Manager when a consumer no longer requires the current level of care, and provide direct services in accordance with contractual guidelines. Adherence to standards of care is essential to ensure that identified quality measures are met. Timely data submission to the Director and Program Manager is required to fulfill reporting obligations. The specialist may also be asked to provide case management, psycho-educational services, and facilitate groups as needed. Regular attendance is crucial for this position, and the specialist will maintain clinical records in accordance with policies and procedures, completing progress notes and treatment plan reviews in a timely manner.

Responsibilities

Requirements

  • Bachelor's degree preferred; in lieu of a Bachelor's degree, an Associate's degree or equivalent college coursework (2 years) and 2 years of work experience in the behavioral health field is required.
  • High school diploma with a minimum of 4 years of experience in the behavioral health field is acceptable.
  • RSS certification, mental health certification, or related training preferred.
  • Knowledge of Psychiatric Rehabilitation is desirable.
  • Must have and maintain a valid driver's license.
  • Must maintain an acceptable driving record and be insurable under the organization's auto insurance plan.
  • Proficiency in Microsoft 365 is required.
  • Ability to navigate through electronic health records (EHR) system.
