About The Position

The Community Support Specialist - Specialized Services position at University Health involves providing direct support and case management services to individuals in their homes and community settings. The position is full-time, with a work schedule from 8:00 AM to 4:30 PM, totaling 40 hours per week.

Responsibilities

• Provide direct support and case management services to individuals in their homes and community settings.
• Work closely with a dedicated and multidisciplinary team, including psychiatrists, nurses, and peer support specialists, to develop and implement individualized treatment plans.
• Assist clients in accessing community resources, including housing, employment, medical care, and social services.
• Advocate for and empower clients, ensuring their voices and preferences are central to all care decisions.
• Help clients develop coping strategies, facilitate skill-building, and provide continuous monitoring and support to ensure sustained progress.
