About the position

The Community Support Specialist role at ReDiscover focuses on providing support to individuals experiencing suicidal thoughts and behaviors. This position is part of the Adult Suicide Prevention team, which aims to connect clients with long-term support while assisting them through immediate crises. The specialist will maintain a caseload of clients, facilitate their achievement of long-range outcomes, and collaborate with a multi-disciplinary team to ensure effective client care.

Responsibilities

  • Maintain and interact with a caseload of clients either in-person or over the phone.
  • Facilitate each client's achievement of long-range outcomes of symptom management, employment, and independent living based on their needs.
  • Engage difficult-to-serve clients by investigating all potential resources to gain access and make connections.
  • Help remove barriers for clients, including participation in community or social service programs, referrals to educational or working programs, and assistance with transportation barriers.
  • Collect and maintain information about clients and document interactions within ReDiscover's designated electronic medical record.
  • Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.

Requirements

  • 4-year degree in a social science field or 4 years of equivalent experience in lieu of degree.
  • A valid automobile driver's license, appropriate automobile insurance, and a safe, reliable, registered automobile available for use during work.
  • At least two (2) years of clinical experience in direct service provision to children and adolescents in a mental health setting preferred.
  • Knowledge of community resources and health and social service systems in the Kansas City metro area.
  • Excellent customer service skills with a recovery focus.
  • Strong written, oral, and interpersonal communication skills.
  • Intermediate computer skills in typing/keyboarding, Microsoft Word, Excel, and GSuite.

Nice-to-haves

  • Experience working with children and adolescents in a mental health setting.
  • Familiarity with community resources in the Kansas City metro area.
