About the position

The Community Support Specialist Supervisor at Preferred Family Healthcare is responsible for overseeing a team of associates who provide care coordination and access to community resources for individuals with mental health and/or substance use disorders. This role is crucial in ensuring that the services rendered are appropriate, clinically justified, and delivered at the right frequency and intensity based on client needs. The supervisor will work under the guidance of the Director or Vice President to establish and maintain effective working relationships with other agencies in the communities served. This includes conducting monthly record reviews to ensure compliance with State Regulatory and CARF standards, as well as monitoring the program's Quality Improvement Indicators. In addition to compliance monitoring, the supervisor will provide oversight for service provision, including conducting spot checks on staff documentation to ensure accuracy and appropriateness of services. They will also be responsible for developing annual training schedules for Community Support Specialists, coordinating required in-services, and monitoring staff completion of annual trainings. The supervisor will play a key role in promoting the overall health and wellness of clients, adhering to the SAMHSA 4 Principles of Recovery: Health, Home, Purpose, and Community. The position requires the supervisor to ensure that community support associates represent the agency professionally and maintain appropriate contact with clients, family members, referral sources, and outside entities. They will also be tasked with developing program schedules, coordinating services with outside agencies, and monitoring the program budget. The supervisor will provide clinical direction and supervision, perform corrective coaching, and maintain individualized training plans for associates. This role is essential in fostering a team approach to increase communication, address complaints, and develop plans to improve program concerns. The supervisor will also assist the Program Director in preparing quarterly reports and indicators, ensuring that all policies and procedures are implemented effectively to guide service provision.

Responsibilities

  • Supervise a team of associates providing care coordination and accessing community resources for individuals with mental health and/or substance use disorders.
  • Establish and maintain effective working relationships with other agencies in the communities served under the guidance of the Director or Vice President.
  • Complete monthly record reviews to ensure compliance with State Regulatory/CARF standards and monitor program Quality Improvement Indicators.
  • Follow up on deficiencies during weekly staffing meetings and assist associates in correcting them.
  • Provide oversight for service provision, including conducting spot checks on staff documentation for accuracy and appropriateness of services.
  • Monitor progress notes for compliance with State Regulatory/CARF regulations and treatment plans.
  • Ensure Community Support associates are equipped to deliver services in community-based settings and monitor service delivery.
  • Develop annual training schedules for Community Support Specialists and coordinate required in-services.
  • Complete and review written reports to referral sources in a timely manner.
  • Promote integration of clinical, community support, and physical healthcare services as a team leader.
  • Ensure community support associates represent the agency professionally and maintain appropriate contact with clients and outside entities.
  • Develop program schedules and ensure client rights and responsibilities are respected.
  • Coordinate services and collaborate with outside agencies to provide comprehensive services to clients and their families.
  • Monitor program budget and review service delivery and expenditures of allocated funds.
  • Provide uninterrupted coverage in accordance with licensing standards by scheduling associates and arranging substitute coverage during absences.
  • Monitor and follow up on crisis calls occurring outside of program hours.
  • Complete evaluations for PFH associates in a timely manner and provide clinical direction and supervision.
  • Ensure all policies and procedures are implemented to guide service provision and recommend changes to the Program Director.
  • Develop and implement a QM measurement system to promote continuous improvement as a team.
  • Assist the Program Director in quarterly reports and indicators, participating in QM meetings.

Requirements

  • Master's Degree in social work, counseling, psychology, psychiatric nursing, or a closely related field with at least one year of professional experience in substance use disorder treatment; or a Bachelor's Degree with at least two years of experience.
  • Demonstrated competencies in supervision and substance use disorder treatment and rehabilitation.
  • At least two years of supervised experience working with adolescents in education, treatment, or social service settings for adolescent programming.
  • Knowledge of case management and rehabilitation methods related to mental health and substance use disorder treatment services.
  • Ability to plan, supervise, and evaluate the work of Community Support Specialists.
  • Experience in providing one-on-one or group services with a rehabilitation focus.

Nice-to-haves

  • Knowledge of the problems encountered by persons with mental health and/or substance use disorders residing in the community.
  • Experience in teaching and modeling coping strategies for individuals with psychiatric, developmental, or substance use issues.
  • Aptitude for developing, coordinating, and evaluating services within assigned programs.
