About the position

The Community Support Specialist for Transition Age Youth at Seven Counties provides therapeutic support to young individuals, focusing on promoting recovery, independence, and social integration. This role involves teaching daily living skills, facilitating community activities, and ensuring a safe and supportive environment for clients. The specialist acts as an ambassador for the organization, collaborating with community systems and providing personal care as needed.

Responsibilities

  • Teaches, models, and coaches activities of daily living (ADL) such as personal hygiene and meal preparation.
  • Teaches psychoeducation classes on recovery topics.
  • Plans and facilitates community-based activities and access to resources.
  • Contributes to a safe therapeutic environment using verbal de-escalation techniques.
  • Serves as an ambassador for SCS and its clients, educating the community about disabilities.
  • Collaborates with community systems on behalf of individuals served.
  • Provides transportation and teaches public transportation use.
  • Provides personal care as needed.
  • Administers First Aid and CPR as necessary.
  • Supervises self-administered medication or administers medication as per protocols.
  • Identifies clients' food and medication allergies and ensures safety.
  • Communicates relevant care information with SCS staff and authorized providers.
  • May take responsibility for specific program assignments.
  • May provide limited on-the-job training to Community Support Providers.
  • Documents services and care provided in accordance with standards.
  • Maintains confidentiality of protected health information.
  • Attends required staff and service planning meetings and trainings.
  • Maintains and enhances skills for working with clients.

Requirements

  • Up to 18 months' training in direct care of individuals with disabilities beyond high school or GED.
  • Completion of all required trainings within 6 months of hire date.
  • One year full-time experience working with individuals receiving mental health services.
  • Formal documentation of ability to function independently as a direct support professional.
  • Ability to provide caring attention to individuals with disabilities, including those with complex needs.
  • Good interpersonal skills and ability to communicate with verbal and nonverbal individuals.
  • Teaching skills, patience, empathy, maturity, and judgment.
