About the position

The Community Support Specialist at Valley BHcare Inc. is responsible for providing community support services aimed at enhancing the quality of life for consumers through individualized goal development. This role involves skill building and motivational support in areas such as employment, education, and community involvement, ensuring that consumers achieve their personal recovery goals. The specialist will conduct assessments, develop recovery plans, and provide both individual and group sessions, while also offering crisis intervention and outreach as needed.

Responsibilities

  • Provide individual and/or group sessions while meeting caseload and productivity requirements.
  • Conduct functional assessments upon admission and update them quarterly or every six months.
  • Develop person-centered recovery plans with measurable and attainable goals, reviewed at least every 3 months.
  • Observe, record, and evaluate consumer progress towards goals.
  • Document interventions according to the psychiatric rehabilitation skill building model.
  • Provide outreach and crisis intervention to community support service consumers as needed.
  • Assist clients with connecting to medical and/or psychiatric appointments.
  • Educate and support family members and community members to meet rehabilitation goals.
  • Develop individualized and generalized community supports.
  • Consult with the Program Manager regarding consumer care levels.
  • Maintain clinical records in accordance with policies and procedures.

Requirements

  • Bachelor's degree preferred; or Associate's degree with 2 years of experience in the behavioral health field; or high school diploma with 4 years of experience in the behavioral health field.
  • Crisis intervention experience.
  • Valid driver's license and acceptable driving record.
  • Proficiency in Microsoft 365 and electronic health records (EHR) systems.

Nice-to-haves

  • RSS certification or mental health certification preferred.
  • Knowledge of Psychiatric Rehabilitation is desirable.
