About the position

The Community Support Specialist for the Veterans Project at Thresholds plays a crucial role in providing mental health and substance use treatment support to veterans in their communities. This position focuses on care planning and case management, helping clients navigate complex systems and access necessary resources. The role requires adaptability to meet clients' varying needs and schedules, fostering their growth and recovery through direct support and transportation to appointments. Responsibilities

  • Provide mental health and substance use treatment support to veterans in their community.
  • Assist clients in care planning and case management to remove barriers to mental healthcare.
  • Connect clients with resources and help them navigate complex systems.
  • Strengthen clients' daily living skills by providing transportation to appointments and other necessary locations.
  • Adapt to clients' schedules, needs, and treatment plans on a daily basis.

Requirements

  • Valid Licensed Clinical Social Worker license (preferred).
  • Master's degree in Psychology, Social Work, or related field (preferred).
  • High school diploma or GED with 5 years of supervised clinical experience.
  • Bachelor's degree in Psychology, Social Work, or related field.
  • Bachelor's degree in an unrelated field with 2 years of supervised clinical experience.
  • Effective communication and relationship-building skills.
  • Strong writing skills and time management abilities.
  • At least 21 years old.
  • Valid driver's license, current car insurance, and daily access to a car.
