About the position

The Community Support Specialist (CSS) at Thresholds plays a vital role in providing mental health and substance use treatment support within the community. This position focuses on care planning and case management to help clients navigate complex systems and access necessary resources, ultimately aiding in their recovery journey. The role requires adaptability to meet clients' varying needs and schedules, fostering a collaborative and supportive environment for both clients and staff.

Responsibilities

  • Provide mental health and substance use treatment support in the community.
  • Assist clients in care planning and case management.
  • Connect clients with resources and help them navigate complex systems.
  • Strengthen clients' daily living skills by providing transportation to appointments and essential services.
  • Adapt to clients' schedules and treatment plans to ensure effective support.

Requirements

  • Effective communication and relationship-building skills.
  • Good writing skills.
  • Strong time management and organizational abilities.
  • Must be at least 21 years old.
  • Valid driver's license, current car insurance, and daily access to a car.
  • Willingness to provide transportation to clients.
  • High school diploma or GED with 5 years of supervised clinical experience, or a Bachelor's degree in Psychology, Social Work, or a related field, or a Bachelor's degree in an unrelated field with 2 years of supervised clinical experience, or a Master's degree in Psychology, Social Work, or a related field.
