About the position

The Community Support Specialist role focuses on providing community support services to young adults dealing with serious mental health challenges. This position involves working directly in the community, assisting clients in accessing resources such as housing and food assistance, and maintaining regular in-person contact with clients to support their recovery efforts. The role requires local travel and the use of a personal vehicle for client transportation, with mileage reimbursement provided.

Responsibilities

  • Provides community support services to complex clients, utilizing treatment plan interventions for positive outcomes based on individual strengths and needs.
  • Completes timely documentation of services, detailing the need for the service, interventions provided, and the client's response.
  • Contacts individuals or referral sources after missed appointments to re-engage clients and promote recovery efforts.
  • Supports individuals in crisis situations and provides mentorship and job shadowing to community support colleagues.

Requirements

  • Bachelor's Degree in Human Services or a related field.
  • Valid Class D (IL) or Class E (MO) driver's license.
