About the position

The Community Support Specialist for the Youth Suicide Prevention Team at ReDiscover plays a crucial role in providing support to youth and adults experiencing suicidal thoughts and behaviors. This position involves maintaining a caseload of clients, facilitating their long-term outcomes, and connecting them to necessary resources and services. The specialist will work closely with a multi-disciplinary team to ensure effective communication and coordination of care, all while focusing on the personal and professional development of the individual in this role.

Responsibilities

  • Maintain and interact with a caseload of clients either in-person or over the phone.
  • Facilitate each client's achievement of long-range outcomes of symptom management, employment, and independent living based on their needs.
  • Engage difficult-to-serve clients by investigating all potential resources to gain access and make connections.
  • Help remove barriers for clients, including participation in community or social service programs, referrals to educational or working programs, and assistance with transportation barriers.
  • Collect and maintain information about clients and document interactions within ReDiscover's designated electronic medical record.
  • Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.

Requirements

  • 4-year degree in a social science field or 4 years of equivalent experience in lieu of degree.
  • A valid automobile driver's license, appropriate automobile insurance, and a safe, reliable, registered automobile available for use during work.
  • At least two (2) years of clinical experience in direct service provision to children and adolescents in a mental health setting preferred.
  • Knowledge of community resources and health and social service systems in the Kansas City metro area, with skills in establishing working relationships with community partners preferred.
  • Excellent customer service skills with a recovery focus.
  • Strong written, oral, and interpersonal communication skills.
  • Intermediate computer skills in typing/keyboarding, Microsoft Word, Excel, and GSuite.

Nice-to-haves

  • Experience working with youth and adolescents in a mental health setting.
  • Familiarity with community resources in the Kansas City area.
