About the position

The Community Support Specialist will be a vital member of the Youth Suicide Prevention team at ReDiscover, a non-profit community mental health center. This role focuses on providing support to youth and adults experiencing suicidal thoughts and behaviors, connecting them to long-term resources while assisting them through immediate crises. The specialist will maintain a caseload of clients, facilitate their progress towards recovery, and collaborate with a multi-disciplinary team to ensure comprehensive care.

Responsibilities

  • Maintain and interact with a caseload of clients either in-person or over the phone.
  • Facilitate each client's achievement of long-range outcomes such as symptom management, employment, and independent living based on their needs.
  • Engage difficult-to-serve clients by investigating all potential resources to gain access and make connections.
  • Help remove barriers for clients, including participation in community or social service programs, referrals to educational or working programs, and assistance with transportation barriers.
  • Collect and maintain information about clients and document interactions within ReDiscover's designated electronic medical record.
  • Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.

Requirements

  • Bachelor's degree in a social science field or 4 years of equivalent experience in lieu of degree.
  • A valid automobile driver's license and appropriate automobile insurance.
  • At least two years of clinical experience in direct service provision to children and adolescents in a mental health setting preferred.
  • Knowledge of community resources and health and social service systems in the Kansas City metro area.
  • Excellent customer service skills with a recovery focus.
  • Strong written, oral, and interpersonal communication skills.
  • Intermediate computer skills in typing/keyboarding, Microsoft Word, Excel, and GSuite.

Nice-to-haves

  • Knowledge of community resources and health and social service systems in the Kansas City metro area.
  • Two years of clinical experience in a direct service provision to children and adolescents in a mental health setting.
